Police Report Details on Crash Holly's death
(November 20) NAVARRE - Santa Rosa County Special Investigators are awaiting a toxicology report for the official cause of death of former WWE Hardcore Champion Crash Holly (Michael Lockwood).
Holly, who also wrestled under the aliases of Mad Mikey (NWA/TNA) and Erin O’Grady in the APW before joining the WWE, was found dead by Santa Rosa County Deputies on Nov. 6.
Deputies were dispatched to the home of Holly’s friend and current WWE wrestler Steven Richards after they received a call from Connecticut State Trooper Holsterom, a friend of Crash.
Holsterom called law enforcement officials on Nov. 6 to check on Holly’s welfare after describing him as depressed on the telephone the night before.
After responding to the residence and finding no one at home, Dep. Robert Arthur reported back to Trooper Holsterom that after knocking, ringing the doorbell and checking all of the doors and windows, he was unable to get any response.
Trooper Holsterom then contacted Santa Rosa County Realtor James Holland, who listed the Richards house and had him get in touch with Deputy Arthur to give him access to the home.
Holland opened the home for Deputy Arthur, who found Holly laying face down in the kitchen partially clothed.
Holly, 32, was pronounced dead at 2:37 p.m. on Thursday and is survived by his wife and daughter.
Holly had a pool of vomit and blood around his face and rigor had set in the body by the time it was discovered.
Deputy Arthur found an open fifth of Southern Comfort on the kitchen counter and an empty pill bottle.
Authorities found the bottle to have contained 350 mg capsules of Carisoprodol, which is a generic medication for Soma.
The prescription for the generic Soma was issued on Nov. 4 with 90 capsules and Holly was prescribed three pills per day, but was found to be empty when officers arrived.
Dr. Andrea Minyard, with the District 1 Florida Medical Examiners Office in Pensacola, conducted an autopsy on Nov. 7 and found no evidence of bruising or other injuries, according to Santa Rosa Crime Scene Technician Charlie Peters.
Dr. Minyard believed the pending toxicology report would reveal the cause of death.
Normally the results of a toxicology report are returned in 12 to 14 weeks according to the District 1 Medical Examiners office.
During a search of the scene, Deputies of the Santa Rosa County Crime Scene Investigations Unit found copy of separation and property agreement forms from Rowan County, N.C., a prescription bottle that contained Effexor 75 mg tablets and a cell phone.
The last number called by Holly was that of Richards, the owner of the residence in which Holley’s body was found, who deputies identified as Mike Manna, on Nov. 5 at 12:54 p.m.
Further investigation of the house led deputies to a computer, which was searched for the possibility of a note or some indication as to why Holly died.
In the garage officers found a green Ford Explorer, which contained a Jansport backpack. Inside the backpack deputies found money, paperwork from Children and Family Services, Santa Rosa County Avalon Center DUI class papers and a prescription.
A brief case in the vehicle contained divorce papers.
Holly was arrested by a Santa Rosa County Sheriff’s Deputy on Oct. 4 and charged with driving under the influence as well as drug possession, according to Santa Rosa County Public Information Deputy Jerry Henderson.

(November 9) Michael John "Crash Holly" Lockwood, 32, of 1215 Steven Drive, Salisbury, died Thursday (Nov. 6, 2003) in Navarre, Fla.
Born August 25, 1971, in San Francisco, Calif., to Barbara Balestreri Albertoni of Pacifica, Cali., and Robert Lockwood, of California, Mr. Lockwood was a graduate of Terra Nova High School.
He was a professional wrestler for National Wrestling Alliance and Total Nonstop Action and was formerly of World Wrestling Entertainment.
He formerly owned and operated Crash Holly's School of Professional Wrestling in Salisbury.
He is survived by wife Christeena Wheeler Lockwood; daughter Patricia E. Lockwood, of Pacifica, Calif.; and brother Mario John Lockwood of California.
Services: 2 p.m. Wednesday at Linn-Honeycutt Funeral Home, China Grove. Burial, West Lawn Memorial Park.
Visitation: 6:30-8 p.m. Tuesday at the funeral home. (November 9)

Crash Holly found dead at 34
(November 6) WWE.com and wrestlingobserver.com have reported that Mike Lockwood, better known as Crash Holly and more recently, Mad Mikey, was found dead this afternoon at WWE star Steven Richards' home in Florida. He was 34.
Lockwood started his career in California, wrestling as Leprechaun Erin O'Grady before joining the WWE in 1999, as the cousin of Bob "Hardcore" Holly. The two frequently battled each other but also went on to win the WWF Tag Team Championship.
He was used mostly as a comedy wrestler, announced as weighing "Well over 400 pounds" despite his small stature and carrying a scale to the ring. His greatest WWE claim to fame was his multiple Hardcore Championship reigns. He was the first champion to adhere to the 24/7 rule and sketches revolving around wrestlers, non-wrestlers, and WWE agents Pat Patterson & Gerald Briscoe trying to steal the title from him were frequently a part of WWE programming.
After floating around on Velocity and being unused by the company, they finally found him a role as one of Matt Hardy's followers, and he began using A.J. Styles' signature finisher, dubbing it the "Crash Landing." He was released shortly thereafter, opened "Crash Holly's School of Professional Wrestling in Salisbury, NC, and and surfaced in NWA-TNA as Mad Mikey, forming a team with Shark Boy.
TNA, Lockwood's most current employers, posted a message condolence on their website. "TNA Entertainment is deeply saddened to report that Michael Lockwood, also known as Mad Mikey and Crash Holly, passed away on Thursday. Our thoughts and prayers are with Mikey's family and friends during this difficult time."
Lockwood was born August 25, 1971 in Anaheim, California. Trained by Jerry Monti, Michael Modest and Ric Thompson, Lockwood made his debut in 1989. He had a long career in the independent scene, including a brief stint in ECW in 1998. His gimmicks in those early days included being known as The Leprechaun, Erin O'Grady and Johnny Pearson. Lockwood also spent time in Mexico in his early days.
While not he was on the Wrestlemania X-8 card, he made the trip up to Toronto to meet and greet fans. He enjoyed interacting with the fans at the event, and applauded the WWF for putting on the fan-fest event. "I think it's a great thing," Lockwood told SLAM! Wrestling at the time. "I think it's the one opportunity that WWF wrestlers really get to be in touch with their fans."
There is no official cause of death as of press time.
